6/23/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Anthony upon arrival to the home. QMHP-C arrived to the home to conduct an individual session with Anthony. Upon arrival to the home setting, Anthony appeared to be in a good mood. QMHP-C informed Anthony that this session was going to focus on being able to interact with his peers and others in an appropriate manner without the use of verbal and physical aggression. 2. Behavior: Anthony appeared to be receptive to the interventions that were provided to him as well as him presenting cooperative behaviors throughout the session; however, at times, he needed a little redirection from QMHP-C as it relates to his verbiage at times. 1.Intervention: QMHP-C began the session by exploring Anthonyâs emotional wellbeing. QMHP-C explored with Anthony and Anthonyâs mother Anthonyâs impulsivity and how it affects Anthonyâs relationships with her family members. QMHP-C encouraged Anthony to utilize âstop, think and reflectâ techniques before making decisions. QMHP-C explored with Anthony and family several scenarios and encouraged Anthony and family to utilize Situation, Options, Disadvantages, Advantages and Solution (SODAS) to determine what to do. 2. Intervention: QMHP-C educated Anthony on impulse control strategies and praised Anthony and family for their participation in the session. QMHP-C facilitated a therapeutic game for Anthony and family to work on together. QMHP-C ended session by reviewing interventions with Anthony and encouraging Anthony and family to provide feedback. 1. Response:. Anthony appeared to be receptive to counselorsâ interventions and services that were provided to him during the session. While exploring Anthonyâs emotional state, Anthony expressed to QMHP-C that he was in a good mood. Anthony stated that he was excited because he wanted to show QMHP-C what he has been writing in his journal since the last session. Anthonyâs mother stated that she was upset because she had concerns about her job and the financing when it comes to her obtaining different housing for her and her children. Anthonyâs mother stated that Anthony and his siblings often do things without thinking about the consequences and that she is consistently yelling at them to clean up and stop arguing with each other. While exploring âstop, think and reflect techniquesâ, Anthonyâs mother stated that this would be helpful if Anthony and his siblings would use it. 2. Response: Anthony and family actively participated and gave several options for solutions to the situation. During the therapeutic activity Anthony and his siblings were yelling at each other, pushing each otherâs hands out of the way and arguing. Anthonyâs mother stated that this resembled how they interact with each other in the home. At the end of the session, Anthony and siblings stated that the game would have been more efficient and completed quicker if they worked together. Anthony then expressed to QMHP-C at the end of the session, âI really liked this Ms. Vaughn, because it has been a long time since we all have just been togetherâ.
